long since i have time to sit down and post...

it has been a busy week... filled with calls and very malaise post-call days... my to-do list is building up again...

granny is not well... and it bothers me more and more... she's been quite well all her life until the beginning of 2007... she's in her eighties, premorbid "lived with family, ADLI, walked with stick"... it started off with a vasculitis and CVA... she then become "poor vision and walks few steps indoor with frame"... only now we know that's just the beginning... the neurologist started her on steroids... it improved her sight but complicated by an osteoporotic compression fracture of T12... the pain was very severe... it does not go away with bed rest + pethidine + physiotherapy... she already experienced pretty bad ileus from the opiods before the analgesic became effective... because of DM + renal impairment she is not fit for NSAID... then because of prolonged bed rest + hospitalisation, a tiny sacral sore is budding, she's becoming delirious... not to mention she's wasted and deconditioned... she's screaming in pain every day...

being a physician trainee, i deal with elderly patients every day... my patients complain of pain here and there... usually i tell them this is degeration and noting much can be done and put down analgesic balm LA bd + dologesic 1 tab qid prn +/- physio x pain relief... it's hard to believe there's pain which cannot be controlled in the modern world... her situation is quite a bad one one can get... this is suffering... what's more, yungyung is the one who brought me up and is my very dear grandma...

now the orthopaedic surgeon offers fluroscopic vertebroplasty with bone cement... apparently my granny and family wants to give it a try since she's in so so so much pain... the risk is small but can be very devastating... it is a benign disease but why is she going through all these suffering... sigh...
